Contact Us
The "Contact Us" page has:
- A title "please enter your message"
- A form with a single element where participants can enter text. A character count is displayed when the message length approaches max character limit.
- A "Send" button. This emails a message to the administrators that have access to the currently selected study, and then redirects to a page with the message: "Your message has been sent to the CTRL study administrator. A copy of your message has been sent to your email address."
- A "Cancel" button that takes the participant back to the "My Activities" page.
- To send a message related to a different study, change the selected study from the "My activities" page, then click back to the "Contact Us" page.
